A new patch is applied and worn for 7 days for 3 consecutive weeks ( change to a new patch every 7 days). No patch is worn during Week 4, when you might likely will have withdrawal bleeding . Only wear 1 patch at a time.
Twirla birth control patch has 2 main active ingredients: Levonorgestrel 120 mcg/day (progesterone) and Ethinyl Estradiol 30 mcg/day (estrogen).
The most common side effects of Twirla include: skin reactions at the patch site, nausea, headache, menstrual cramps and/or weight gain. These are not all of the possible side effects of Twirla.
The chances of serious side effects are rare. Like pregnancy, hormonal birth control may increase the risk of serious blood clots especially in women who have risk factors, such as smoking, high blood pressure, high levels of cholesterol in the blood, diabetes, obesity, a family history of blood clots or are older than 35 years old. These may sound scary, but remember — those side effects are rare.

Twirla is a combination birth control contraceptive patch with 30 mcg estrogen and 120mcg levonorgestrel for women with a BMI less than 30 kg/m2.
Twirla may not be as effective in women with a BMI of 25 kg/m2 or more. It lowers the risk of becoming pregnant by preventing the release of an egg from the ovary (ovulation). In addition, combination birth control also thickens the cervical mucus, helping to block sperm from getting into the uterus and getting to the egg.
Do not use Twirla if you smoke cigarettes and are over 35 years old. Smoking increases your risk of serious cardiovascular side effects from combination hormonal contraceptives (CHCs), including death from heart attack, blood clots or stroke. This risk increases with age and the number of cigarettes you smoke. In these cases, Pandia Health Medical group’s doctors recommend progestin-only pill (POPs), IUD with hormone (Mirena, Liletta, Kyleena, Skyla), implant (nexplanon), the birth control shot (depo-provera), or condoms and spermicide.
Do not use Twirla if your body mass index (BMI) is 30 kg/m2 or more. Check your BMI here. Women with a BMI of 30 kg/m2 or more who use combination birth control have a higher risk for developing side effects like blood clots compared to women with a BMI lower than 30 kg/m2.

Twirla is a 28 cm2 circular patch and Xulane, the generic brand of Ortho Evra, is a 14 cm2 square-like patch. Zafemy is another generic to Ortho Evra and is a 12.5 cm2 square-like patch. Additionally, Xulane and Zafemy have a different combination of active ingredients and a higher delivered estrogen dose than Twirla.
Twirla can be applied to one of 3 locations on the body; the abdomen, buttock or upper torso excluding the breast. Do not put the patch on your waistline or near clothing undergarment seams. Do not put the patch on the breasts, on cut or irritated skin (rashes or other skin problems), or on the same location as the old patch.
Twirla is a combination birth control contraceptive patch for women with a BMI less than 30 kg/m2 who may become pregnant. Twirla may not be as effective in women with a BMI of 25 kg/m2 or more. It is a patch containing two hormones called estrogen and progestin. These two hormones primarily work by stopping the release of an egg from an ovary.
